Position Overview:
We are currently looking for an RN MDS Coordinator excited to have the autonomy to coordinate and manage the overall nursing care delivered to residents in our Skilled Nursing Facility. The MDS Coordinator ensures services and care are appropriate to meet residents’ needs. This position is responsible to oversee and coordinate the Resident Assessment Instrument (RAI) and Minimum Data Set (MDS) process. The position includes managing and completing necessary assessments, care planning, and participating in care conferences. This position is a wonderful opportunity for a talented RN to be involved in both the administrative and direct patient care of nursing!

**This position requires high-clinical competence**

- Ability to work with residents, their families, staff, and the public on a personal basis under all conditions.
- Responsible for developing and maintaining resident assessments, care plans, and quality management program in accordance with current rules, regulations, and guidelines that govern the MDS process. Ensures that each portion of these assessments are signed and dated in timely manner to certify the completion.
- Collaborate with administration and management staff to update policies and procedure including implementation of resident assessment (MDS), care plans, discharge process, and attend facility required committees
- Responsible to conduct oral and written reports to our Director of Nursing on nursing care to residents.
- Participate in facility surveys (inspections) made by the authorized government agencies.
- Identify and implement nursing service educational needs.
- Ability to assume tasks of the Resident Care Manager as needed in assist in assessing and evaluating the resident’s change of condition, appropriate interventions, diagnostics, and therapeutic procedures and treatments.
- Attend and participate in annual OSHA and CDC in-service training programs for hazard communications, TB management, and bloodborne pathogens standards.

- Experience and training with RAI/MDS process
- Active CA RN License
- CPR (BLS) Certification
- Skilled Nursing Facility experience preferred
- Knowledge of State and Federal Regulations
- Clinical management or supervisory skills
