SUMMARY: Responsible and accountable for managing the nursing care to Independent and Assisted Living residents. Oversees the delivery of services and staffing for the Clinic and Assisted Living. Works with physicians to implement appropriate care for residents. ESSENTIAL FUNCTIONS: Provides supervision of and performs nursing assessment and treatment to residents during clinic hours; residential and Assisted Living apartment visits; responsible for assessing physical, mental and emotional needs of independent and assisted living residents. Interviews, hires, supervises, trains, motivates, and evaluates performance of staff. Accountable for supporting the attainment of performance goals as measured by specific indicators in Financial, Marketing, Productivity, Risk Prevention and Resident Satisfaction segments of the business unit. Responds to emergency situations and expedites indicated treatment; secures follow-up assistance, as appropriate. Communicates with and assists the physicians to ensure that changes in treatment are processed; residents are assessed and evaluated appropriately for medical, emotional and nursing needs; appointments with physicians are made; and oversees clinic visit scheduling. Oversees and administers, as necessary, medications and treatments according to physiciansA cents € (TM) orders. Consults family members when indicated to ensure that residentsA cents € (TM) needs are met. Participates in the evaluation of the need for permanent or transient stays at all levels of care including hospitalization, assisted living, or skilled nursing. Informs Director of Nursing Services of any events which may necessitate intervention. Coordinates the functions and processes of a Clinic environment including systems, space utilization, supply and equipment availability, and manages cost containment issues. Responsible for maintaining residentsA cents € (TM) medical charts. Assesses the need for additional help or services for residents; coordinates with residents, their responsible party and/or family members and with other departments, as necessary, to meet needs. Assures that private duty aids retained by residents satisfy NCPHS criteria. Monitors the service plans of residents with private duty aids. Assists in the coordination of discharges and admissions of Independent, and Assisted Living residents to the Health Center; tracks Independent and Assisted Living residents in acute care and communicates information with Nurse Manager A cents € " Skilled Nursing Facility and Director of Nursing Services. Coordinates monthly Resident Review. Responsible for maintaining service plans and all other DSS required forms. Schedules physicians and regular staffing and coordinates replacement staffing as needed. Responsible for all residential care, Independent and Assisted Living Continuous Quality Improvement activities by ensuring nursing domains are monitored and reports are prepared. QUALIFICATIONS: Graduated from an accredited school of nursing and licensure to practice as an RN in the State of California. BSN degree preferred. Experience equivalent to a minimum 2-5 years nursing experience which includes supervisory responsibility in a geriatric setting. Basic Life Support certificate. Dementia experience and RCFE training preferred. SUPERVISORY RESPONSIBILITY: Supervises up to 30 staff in the Clinic and Assisted Living unit.
